At least 3 years of experience required for all positions.

  1. Driver's License
  2. US Social Security Card OR ORIGINAL Birth Certificate
  3. Social Security Numbers of all dependents.
  4. Birth Dates of all dependents.
  5. Date of Marriage/Divorce/Widowed
  6. Insurance Card from previous Provider if applicable (or cards to cover last 24 months)
  7. Current MSHA 5000-23 Annual Retraining
  8. Application
  9. Miner's Card / Blaster's Card / Foreman's Card / EMT Card / etc...
  10. Social Security #'s, Date of Birth, and Addresses of any possible Life Insurance Beneficiaries (including this information for those who may receive the benefits if the primary beneficiary becomes deceased AND also for the adults who would be in trust of any minor children)
  11. A check from a Personal Checking or Savings Account if you would like your payroll check direct deposited instead of mailed to your residence
